1.) Her heart rate stays the same and she's not breathing heavily. A woman in the true throes of a mind-blowing orgasm will have a heartbeat as wild and racy as a runaway train. If she's not faking it, her body will react by increasing her heart rate and respiration. It will be involuntary and is not something she can control. So if she's consistently panting away, you've hit the right button.

2.) You don't feel the contractions. If you don't know what this means, then you've never taken a woman to her happy place. If she's not faking it, there will be distinct contractions in the kitty area, almost like she's doing mini-clenches on your penis. This is not the kitty grip that women do from their kegels, but rather involuntary spasms. They're not necessarily strong ones, but they will be noticeable.

3.) Her skin is cool to the touch and not flushed. Body temperature is one of those giveaways we can't consciously moderate. A nice juicy orgasm will make a woman's temperature increase and make her skin flush a nice rosy color. If she's cool to the touch throughout the love fest, she's faking it.
